Lake City is pleased to now welcome visitors! All visitation will be conducted in accordance with guidelines and recommendations issued by the Centers for Medicare & Medicaid Services (CMS), Center for Disease Control (CDC), and the state and local health departments. Please contact our Activities Department for further information and to schedule a visit.
Do you have beds available?
Bed availability can change on a daily basis. Please contact the Admissions Department for availability.
Do you have Social Workers to assist with discharge planning?
Yes, upon admissions, our social worker will meet with you and discuss the plan of care for the resident. They will follow up on the progress throughout the care plan and will meet with you to assist with discharge planning and initiate any home services desired.
How do I admit a loved one from the hospital?
Inform the social worker at the hospital that you desire admission to Lake City Nursing & Rehab. They will contact and fax the referral to our Admissions Department
How do I admit a loved one from home?
There are certain forms that need to be completed by the primary care physician including a History and Physical, State OBRA screening, admission order and a list of medications. The physician’s office will be familiar with these forms but please contact us; we would love to assist you.
With physical, occupational, speech and outpatient services, you can trust that Lake City Nursing and Rehabilitation Center has all the tools necessary to help our residents reach their maximum potential including our Activities of Daily Living (ADL) suite, equipped to duplicate home activities.

Full Time Respiratory Therapist on Staff. Treatment of: tracheostomy management and weaning, Asthma, Chronic Bronchitis, COPD, Pneumonia, Pulmonary Hypertension, Emphysema, CPAP/BiPAP therapy for sleep breathing disorders, and Airway management.
